 
1.  Lost License  --  No address or other changes:  
 
Write the FCC a letter indicating that your license was lost (explaining
the circumstances) and that you wish to obtain a replacement.  Include
your full name, address, call sign and date-of-birth.  Send to FCC,
1270 Fairfield Rd, Gettysburg PA 17325-7245 (do not call them to request
a replacement).  You may fax the request to 717-337-1541.
 
Keep a copy of your letter for your records.  FCC has told us that
if you possess a copy of your letter to the FCC, it will serve as
authority to operate until the replacement license arrives.
 
If you are upgrading soon, you can also request that the FCC send you
a Form 1010-B License Verification Letter which will also serve as
operating authority until the replacement license arrives.
 
The replacement license should come through in four to six weeks.
The Form 1010-B should arrive within two to three weeks, or sooner.
Fax requests save two to three days mailing time...

a completed FCC Form 610.  Send to FCC, 1270 Fairfield Rd, Gettysburg 
PA 17325-7245 (do not call them to request a replacement).  
 
Keep a copy of your letter for your records.  FCC has told us that
if you possess a copy of your letter to the FCC, it will serve as
authority to operate until the replacement license arrives.
 
If you are upgrading soon, you can also request that the FCC send you
a Form 1010-B License Verification Letter which will also serve as

Note: The FCC requires a copy (or the original) of an applicant's 
*current* license (Form 660) to be attached to each upgrade application.
Upgrade applications may not be submitted to the FCC by a VEC until
a copy of the applicant's current license is attached.
 
For example:
 
If a person possess a Novice license, has a Technician upgrade
application pending at the FCC, and then upgrades to General class.
The VEC may not submit the General upgrade application to the FCC
until the Technician license is issued--after which the applicant
sends a copy to the VEC for attaching to the General class upgrade
application.  The Technician license is the current license in 
this example.
